We are excited to offer our enrollment package on-line for your convenience. Below you will find the links to all of the forms required by Southside United Methodist Preschool and the Department of Children and Families.
A NEW ENROLLMENT FORM AND FLU FORM, MUST BE COMPLETED EVERY SCHOOL YEAR.
NEW IMMUNIZATION AND HEALTH PHYSICAL FORMS ARE NOT REQUIRED AS LONG AS THEY ARE NOT EXPIRED.
BIRTH CERTICATE IS NOT REQUIRED IF WE CURRENTLY HAVE ONE ON FILE
Please download and print each REQUIRED form, complete and turn into the Preschool Office nO LATER THAN aUGUST 18TH.
You may also leave your forms in the church office in “preschool document box”.
Due to Summer hours for both church anF preschool, Please call prior to coming to make sure someone is available.
All Forms and payments should be in by August 18th
Besides the Required Forms attached below, 3 additional forms will be required:
1.) Florida School Entry Health Form (form 3040) - obtained from Pediatrician, Health Department, Care Clinic.
2.) Florida Certificate of Immunization (form 680 or 681) - obtained from Pediatrician, Health Department
3.) Birth Certificate - original or photo copy (if we do not currently have one on file)

Know your Childcare Center form does not need to be turned in. There is a section on the enrollment form that you will sign acknowledging that you have read it.

carpooL
This service is currently not being provided due to construction in the front parking lot. We hope to offer Carpool at a later date when construction is completed.
Volunteer form is RECOMMENDED for all parents . Required to be on file in the event of participation in any Volunteer opportunity.
